Housing Case Manager
Responsibilities of the Housing Case Manager is to provide services to adults who have a diagnosis mental illness, behavioral health issues, substance abuse, HIV/AIDs, and chronic homelessness. Specific duties included:
Case management services including benefit entitlements and case management.
Securing safe and affordable housing along with completing the necessary financial paperwork for a rental stipend.
Teaching the necessary skills to maintain stable housing.
Developing linkages to community services; and
Develop support plans for clients to utilize community resources when they need services or support.
Requirements
Skills/Qualifications for the Housing Case Manager include:
College degree in human services and/or equivalent experience in the human service field especially in the populations diagnosed with mentally illness, behavioral health issues, substance abuse, and chronic homelessness.
1 year case management experience.
Valid and insurable driver's license.
Both the ability to work independently and as a team member.
Excellent communication skills both verbal and written; and
The ability to be organized and have good time management skills.
Benefits
Salary and Schedule of the Housing Case Manager:
40 hours per week. Monday-Friday.
DAY TIME HOURS AND NO ON-CALL.
$21.87/hour. $45,489.60/ annual equivalent.
Overtime is available in our group home setting, $32.81/her hour for overtime pay.

Supported Housing Case Manager will have an average caseload of 25 clients.
